This information is provided to ensure transparency and to support your rights as a consumer of mental health services.

In accordance with Texas Health and Safety Code §181.105, as amended by House Bill 4224 (89th Regular Session), Vita Counseling Center, PLLC, provides the following information regarding access to health care records, regulatory contact information, and complaint procedures. Mental health services are provided by Rebecca Flores, LPC, a licensed professional counselor. This information applies to services governed by Texas law.

Requesting Your Health Care Records

You have the right to request access to your health care records maintained by Vita Counseling Center, PLLC. Requests for records must be submitted in writing and may require completion of an authorization or records request form in accordance with applicable state and federal laws.
